A tuberculosis contact investigation in a long-term care hospital

Abstract
In 2017, approximately 42% of newly reported tuberculosis (TB) patients were aged 65 years or older. The number of TB contact investigations being conducted in long-term care hospitals has also increased. Here, we reported the results of TB contact investigations in a long-term care hospital. A 90-year-old woman who had been hospitalized in the long-term care hospital was diagnosed with pulmonary TB in May 2016. A total of 308 contacts including staff were evaluated through contact investigations using chest X-ray and interferon gamma release assay (IGRA) test from May 2016 to July 2017; Of the contacts, eight new TB cases were detected additionally and five of these patients were hospitalized in the same ward for the infected persons. Of the 118 contacts tested for IGRA, 31 (26.3%) turned out to be positive. Genotyping results demonstrated that three of the nine patients with positive culture had been infected by the same strain showing an identical pattern.
